If you or your child received an H1N1 flu vaccine at a clinic or location other than one of Harvard Vanguard's practices, please complete this form and submit it to your Harvard Vanguard healthcare provider. Doing so will ensure that your electronic medical record is updated and contains a complete immunization history.
