Enrolling in MyHealth Online
To use MyHealth Online, you’ll need:
- to be at least 18 years of age
- a primary care physician in one of our practices
- internet access and a working e-mail account that you check regularly
- Microsoft's Internet Explorer version 4 or higher, Netscape version 4.0 or higher, or Firefox as your web browser
- a browser set to support 128-bit encryption, and
- to accept the Terms and Conditions statement
Enrolling is easy:
- Fill out an online application.
- Within 3-5 days, we’ll send you an access code.
- Come back to our site, and enter the access code that you receive from us. You’ll then be prompted to create your own username and password.
That’s it. You can then use your username and password each time you visit our site to enter MyHealth Online.

MyHealth Online is not intended to replace an office visit. It should only be used for routine and non-urgent matters. Please do not send messages through MyHealth that require urgent attention. For any urgent medical matters, please contact your clinician’s office. |
