Referrals
Some types of health insurance may require a referral be approved by your 
primary care provider before consulting a specialist.
Your primary care provider will help you in selecting the appropriate specialist for your needs and the patient services/referral coordinator will complete the necessary forms and fax the approved referrals directly to the specialist.

If your insurance requires a referral from your PCP, please have this approved before your specialty appointment.
