Harvard Vanguard Hospitalists

Your Primary Care Physician (PCP) sees patients in the office, but not patients who are in the hospital. This allows your PCP to concentrate exclusively on office-based clinical responsibilities.

Harvard Vanguard has longstanding relationships with many of the best hospitals in the region. Should you require hospital care, a "hospitalist" physician will attend to all your healthcare needs when you are admitted to the hospital.

What is a hospitalist?

Hospitalists are physicians who specialize in hospital medicine. They manage inpatient medical problems and will coordinate your care with various specialists.

What does this mean for you as a patient?

It means that at Harvard Vanguard, you receive coordinated care. Every hospital has hospitalists — but having one who is part of Harvard Vanguard means the hospitalist can access your outpatient medical records by computer and communicate with your PCP about your hospitalization on a regular basis. Together they can determine the goals of your hospitalization.

Once these goals have been reached, the hospitalist will facilitate discharge planning and communicate to your PCP any medical issues that require follow-up. All of your hospital records are available to your PCP in a timely and accurate way. This ensures a smooth, seamless transition as you move from the hospitalist back to your PCP.

If you are hospitalized at a location where there isn't a Harvard Vanguard hospitalist on site, please request that the hospital staff call your primary care physician to inform him or her of your hospital admission. Your primary care physician and a nurse case manager will then be able to follow your progress and coordinate your care after discharge.
